2. Once your orders are confirmed, enter their Order ID numbers into the boxes below. Fill in other details.

Terms and Conditions Both order ID numbers must correspond with valid orders placed no earlier than November 25th, 2002 and no later than January 1st, 2003. Orders must not be flagged as 'high risk of fraud' orders. Subject to telephone verification, and credit card approval/authorization form signature for both orders placed.
